GTX 680 stuck at 705MHz GPU clock

My EVGA GTX 680 SC Signature says it's stuck at 705MHz when using the OSD on EVGA Precision X. I'm unsure if its a bug or if I'm bottlenecking my GPU somehow. I want to play Witcher 3 to its max potential and I'm unsure if I am being able to or not. Any help would be appreciated.
I also tried OCing my GPU with 150+gpu clock + 500 mem clock but its still stuck at 705MHz.

Edit: My memory clock also seems to be stuck at 3602

Specs:
CPU- i7 3770k
RAM- 16gb
240gb SSD
GPU- http://www.evga.com/products/pdf/02G-P4-2683.pdf
GPU driver- 364.72

First use this to uninstall your drivers completly.(will require restart)
The install the latest drivers from NV.

I have Witcher 3 running, its one of the most taxing games on the market right now, so I doubt that's it.

Worked perfectly. It gives the correct reading now and I can see both gpu clock and mem clock fluctuate and decrease.

 

I seem to have another issue now. My EVGA Precision X keeps on freezing whenever I try to alt tab back into it. Any clues? I'm going to try and reinstall it as well.

Nevermind that it's from MSI it doesn't matter, and it really is considered the best GPU overclocking software and most others are based on it(probably even the EVGA one).
