For gtx 750 ti it says,
Maximum Digital Resolution 4096x2160
Maximum VGA Resolution 2048x1536 in specifications section

it means that.
the gtx 750 ti can output 2048x1536 resolution if its connected to the monitor from the VGA port

if its connected through the DVI or HDMI port, the max resolution is 4096x2160

No. it will only output that resolution. the gtx 750ti struggle in maxing out games at 1080p so leave 4k. For 4k gaming you will need at least a r9 390/ gtx 970. gtx 980ti is recommended

yeah the gtx 750ti supports 4k monitors.

You have the DVIs backwards. A digital interface will support the 4k setting. VGA, DVI-A, and DVI-I if you are using the analog half of it will support the smaller resolution.

Not even close. It means it will be able to output that many pixels. Desktop/office use will be fine. But gaming at that res? Not a chance. Doesn't matter the inch on the monitor, if it's 4k and you are using one of the digital interfaces then you're fine. 27" or 55", doesn't matter.
