So the issue is that the graphics card is recognised as a GTX 680 instead of GTX 760
This causes Nvidia to install drivers for 680 instead of 760 and it crashes in games

The card is outputting video and Desktop is stable But games crash due to wrong drivers

Solution

try using different driver. probably better using DDU to uninstall your current driver before installing the new one.
I don't 5hink downloading drivers for GTX680 instead of the one for GTX760 would cause an issue since nvidia for a very long time using unified driver system. Not to mention both GTX760 and GTX680 is based on the very same chip (GK104). Only that GTX760 is based on cut down version of GK104. in the past i did swap my GTX660 to GTX960 without uninstalling my driver first and it did work just fine. And that is between GPU that have different architecture.

My advice is instead of letting your system to automatically download the proper driver for you just download the driver yourself over nvidia.com (it doesn'thave to be the latest one). Your system probably attempting to download older driver for GTX680 when GTX760 still does not exist. Hence that driver might have some issue to recognize your GTX760.

i tried that. installing gtx 760 drivers from nvidia manually. then after this card shows the name as gtx 680 in gpu-z with specs of the zotac gtx 760 which i have 993mhz boost to 1059 and mem clock 1502 😀 . then it crashes in games. looking at the chip its G104-225-A2..
