Question GTX 960 crashing while playing games ?

Hi,

So this started in last 4 days, my games get stuck randomly ( frame freezes) but the background game is ongoing. I have tried reinstalling drivers and noticed that the fan speed and temperature of gpu also drops when this frame freeze happens (using hwinfo64).
This is not particular to any game and is happening in all the games. Some games like witcher 3 just crashes and close while some games like ESO, frame is frozen and everything in background is ongoing. I believe my GPU is dying (it's 4 years old). Otherwise, computer works flawlessly without an issue. Also, sometimes on computer boot up, it shows a blue screen with error video TDR error. Some games also crash with this same error. I am planning to upgrade in Feb but would be sweet if I can stretch and use this GPU till then. My processor is i54400.

Sorry, forgot to include specs -

Proccessor - Intel Core i5-4440
Motherboard Model: MSI B85M-G43 (MS-7823)
Ram r: Corsair 800.0 MHz (DDR3-1600 / PC3-12800)
PSU: not sure about this... 450 watts, don't remember model and brand.

It is crucial to know. Open your case and take a look at the sticker on it.
