Question GTX 960 Problem

Recently got a new case and i have moved everything in to the new one and and connected everything up and my resolution is locked at 800x600 and its showing white lines over my screen and i have no idea whats happened.

monitor connected to the gpu and not the motherboard output?

Monitor is connected through hdmi

Processor: Intel i7 4760k
Cpu Cooler : Corsair h80i
RAM : G.Skill 32GB (4x8GB)
Motherboard: MSI Gaming 5
Graphics card: EVGA GTX 960 2GB
Storage: WD Black 2 TB HD
OP: Windows 10
Power : Corsair 500w
Case (new): Metallic Neo Series ATX silver
 
Could be a few things. Can you test that monitor on another desktop / laptop / tablet? If it works with something else that's probably not your monitor. You could try running your monitor on your integrated graphics to see if it does the same thing. Could be the GPU. Could be the cable of your monitor. Start the elimination process.

Could you please add your monitor to your specs.

I've tried two different monitors and different HDMI cables and the integrated graphics unfortunately
