GTX 970 OC Black Boxes

Hi guys I have the GTX 970 Gigabyte ( not G1) and a gtx 650 which i use for phys x.
I OC'd the 970 to
+10(mV)
power limit on 110%
Core clock +185
Memory +300
using the msi afterburner.
Driver version 355.98

Now the problem occurs when i try to play Witcher 3 , i get Black boxes flashing for a split second , not the tiny ones but like 1/6 of the Screen (23" 1080p).

I dont know if the GPU is dying or did i overclock it too much.
Any help will be appreciated and if possible any guidance.

Sounds like you may be pushing to much for the OC. May want to back things off a bit at a time to see if things change.
