Hard disk's 100% disk usage

I've laptop hard drive Toshiba 320 GB 7200 RPM, the issue is that when I installed OS on it, it shows 100% disk usage. I've installed 8.1 and 10 to check maybe is any issue with OS but its not. I plugged it into another laptop but still same issue. I removed all my partitions but still same issue.
I've connected it to other laptop by using hard disk case to copy my data. at that time it seems normal copying no slow down issue.
Do I need to buy a new hard drive or still there is some work around for it?

this problem is about bad sector i had the same until i got a new hard disk. if you pay attention to the hard disk usage you can see that it has 100% activity while the Read and Write speed are 100 KB so weird right?
it's not about OS it's not about your SATA port or any thing else the only way to fix this is to use it as an backup storage and get a new one .if your using laptop in shocking conditions i recommend to use a SSD with small size to host your operation system and use the HDD you have right now as a secondary storage for storing files and backups
