Hard Drive 100% Spikes

My hard drive is running at 100%. I have tried messing with cables, disabling superfetch and search service, and running a defrag. Sometimes it runs at 100% from startup no matter the load but the load wont reach over 15mb/s typically. Other times it seems to work properly reaching speeds of 45mb/s with consistency and lowering from 100% load. The issue seems random and the issue can last for an extended amount of time, even through resets, the hard drive is quite old. task manager/processes (show CPU, GPU, RAM, DISK, NETWORK) should tell you which process(es) are responsible for the constant reads/writes....

(see if the reads/writes subsist with network disabled...; Glasswire will show you what all network traffic is inbound/outbound)
 

Nothing is using an excessive amount of disk which is the problem, even the system drawing 1.9mb/s can max out the drive.
