Hard drive always at 100%

Basicly ever since I got my new pc, which was a year ago I had some other motherboard and psu
because they burned out after a week, so here are my new pc specs.
hard drive: TOSHIBA DT01ACA100 ATA device 695 gb free of 931
R7 260x
amd fx 4300
8 gb of ram
cx 500 psu
gigabyte 78lmt-usb3 motherboard

every since I got my pc my disk usage would go to 100% almost all the time, most of that time the program that would be using the highest disk usage would be only around 0.1-4 mb/s, but sometimes when I see a program using 60 mb/s its only around 50%. I ran 4 scans with avast premium, malware bytes, and super anti spyware, is the hard drive failing?
 

Use Resource Monitor to find out, what's causing 100% disk usage
(open Disk tab, Disk Activity section, order by column Total, and make File Names column fully visible).

It should look similar to this, just fix ordering by Total:

Usually 100% disk usage is caused by - windows updates, disk defragmenter, windows defender or windows indexing.
 
I've been having the same issue on one of my laptop machines. Disk usage randomly goes up to 100% during gaming or during regular use and everything freezes up. Its a 7200 RPM Drive and not an SSD. Windows came pre-installed with no disc, so I'm not sure how to go about switching out the hard-drive with an SSD. I believe this is a common problem with windows 10/spinning drives. I'm not sure what to do either. The laptop is an alienware 15 r2.

I did notice that prior to the disk going to 100%, I often see "Intel(R)Dynamic Platform and Thermal Framework Utility Application" using up about 30-40% of CPU usage. I've ran anti-malware, ant-virus, anti-root kits, and system file checks. On my to-do list is to run a disk check overnight...but it would be inconvenient for me to tie up the machine for more than a night's sleep. Any help appreciated.
