Hard Drive Fragments itself all the time

hi i Defragmantated My disk drive, after that I search for more fragmentations on the drive and then the disk was fragmented 4%. I need some tips for what I need to do

I recommend using once weekly or even just bi-monthly using either Auslogics disk defragger or Piriform's disk defragger -- they give better reports as what's what before and during and after the defragging.
