hard drive hdtune error scan result

Problem:
I had been getting very slow response times on the most of trivial of tasks like refreshing the desktop or opening my computers and a huge delay in starting windows (WIndows 7 x64 ultimate sp1 ) . (Once while starting my pc, i got a "chkdsk", and it produced a no. of "retreiving orphaned files.....-i dont really know how much significance it was, but i havent seen such things in a chkdsk ever before)
So i tried formatting my hard disk and reinstalling OS, which got stuck for a long time at 46% , and then totally stopped at "Preparing Desktop". The 2nd time i tried installing,it completed, although the windows startup took an awful lot of time.
From then, startups have been real slow. Sometimes my pc did things at the speed it's meant to be. I have played gta v,battlefield 4 with a smooth framerate in the said timeframe,although i noted during the game autosaves, the game froze. Until last afternoon, when it became ABSOLUTELY UNWORKABLY SLOW.
My suspects included my RAM and Hard disk( i have 2 hard drives, 1st- 1TB WD blue where my OS is installed 2nd- 160 gb seagate data drive) To check whether there was any problem of power supply, i disconnected my 160gb drive and the dvd drive. I ran memtest86, 4 passes, zero errors, so ram was ruled out(i guess).
Then i downloaded HD tune and ran on my pc to find out the following results.

0.5% damaged blocks,but health screen indicated OK on all parameters.As the HD tune website tells me, red blocks mean i should backup and REPLACE my hard disk.
So, my question is IS MY HARD DISK REALLY THE CULPRIT IN MY PROBLEM? And SHOULD I REPLACE IT OR THERE IS SOME OTHER WAY? (I cant RMA it, my 2 year warranty expired Aug,15)
My specs: Intel dh61 ww classic mobo
I5 3330 3.0 Ghz processor
8gb ram (2*4gb ram 1333mhz)
Asus strix gtx 960 4gb
Corsair VS550w
PLEASE PLEASE REPLY ASAP as i really need to access my pc.

Sorry that you are facing some issues with your WD drive. :(
It's good that you've backed up the data stored on it.

You've done some extensive troubleshooting. Unfortunately, it seems that there are some bad sectors on it. My guess would be that the HDD is the culprit of the problem you are facing.

It will not hurt to test the drive with WD's Data Lifeguard Diagnostic tool(both short and extended tests): http://products.wdc.com/support/kb.ashx?id=RWZ6VZ

If you get similar results, you will need to replace the drive.

Everything you described can be experienced when your HDD is failing. The fact you are getting damaged sectors when you run HD Tune only further backs this up.

The first thing you need to do (if you have not done so already) is backup all your data.

Once you have done that you have 2 options:

1. Buy a new hard drive and reinstall everything.

2. Run check disk on your failing drive and get it to find and fix errors and locate bad sectors and recovers readable information. This may fix the problem but you need to keep monitoring your drive. If the drives gets more errors then it is time for a new drive but if no more appear then you may get a bit more time from this drive.

The check disk command is chkdsk /f /r

Thanks Rabmac. (y) Yes i had backed up everything of my 1tb WD on an external hard drive. So what about the health part of the hard drive hd tune scan? It says OK, for which i am getting sceptical :3 Any other views guys??
