Yes.
There are 3 basic thicknesses of 2.5" drives.
7mm, 9.5mm, and a few 15mm (large capacity HDD).

The spacer is to ensure a 7mm will fit in a laptop built for a 9.5mm, and have it not move around.
Some drives come with the spacer, to use if needed.

In a desktop I have never needed one due to the way the drives mount, or through the use of a 2.5 to 3.5 adapter.

In laptops I have needed to use something similar, or have used foam tape to take up the space.

Desktop, never needed it.
Laptop, it depends on the specific space in the laptop.
