HDD at 100% active time but no read/write while gaming

So basically, I have this HDD that I just replaced with Seagate a week ago. So since my main SSD was getting filled up, I transferred my games and movies to the SDD. I tried launching Fallout 4 from the HDD but it was loading very slowly in the initial loading screen, so I presumed something was wrong. I pressed ctrl+alt and switched to the task manager, I opened Disk 1(HDD) in the Performance tab. It showed 100% active time on my disk but the read and write speeds were both 0. I have included a few screenshots.

There are a couple things I tried to rectify this issue:
1) I ran chkdsk /b /f /v /scan d: (screenshot attached of results or go to here).


2) I downloaded HDTune and ran it's error scan (screenshot attached of results or go to
here and here and here

3) Then I proceeded on to test both my SSD and my HDD with another game (GTA V). It ran absolutely fine on the SDD but on the HDD it froze for one minute and while starting up it had 100% active time but 0 read/write here

Now I JUST got this HDD one week ago so it cannot have failed so fast. I do not understand whats wrong, I have an iBall PSU with PEAK WATTAGE as 500W so it is really bad and unreliable.
What should I do?



Current build:
CPU: AMD fx6300 base clock
GPU: SAPPHIRE AMD Radeon r7 250x 1 GDDR5 RAM
PSU: iBall PEAK 500W
Motherboard: Gigabyte ga-78lmt-usb3
Case: Some random unnamed one
SSD(boot drive): Samsung EVO 250GB
HDD(secondary storage drive, the one that is not working and is mentioned in all the tests above): Seagate Barracuda 1TB 7200RPM HDD

A truly amazing job with the troubleshooting so far! I'd suggest you try to reseat the connections to the HDD and even try using a different SATA cable and plug the drive in a different SATA port on the motherboard. If the HDD performance does not improve, you should use its manufacturer's brand-specific diagnostic tool and check up on the health and SMART status again. From the looks of your screenshots, the utility that you have used is unable to detect and drive properly and isn't listing all the SMART attributes.
It's possible that the HDD might have failed, so it's better to contact the HDD manufacturer's customer support and let them know about your issues with the hardware.
