HDD has been disappearing randomly since I installed SSD

Ever since I installed my new SSD my old HDD has been crashing and disappearing randomly. I tried formatting it and it helped for a few days then it went back into doing it. I can get it to re-appear if I shut down and turn my PC back on, but eventually I will get the same problem. Any ideas on how I can fix this?

Run crystaldiskinfo and post a picture of the results.

Sounds like the HDD is dying , I would replace it.

Also check the cables.​

Hopefully not. I replaced the SATA cables and nothing changed.

It's dying. Backup and replace it ASAP!
