Unfortunately, this definitely doesn't sound like a healthy drive.
Loud grinding noises usually indicate that an HDD is experiencing an internal hardware failure. This is something that cannot be fixed.
The best thing you can do is backup all your important data from this hard drive somewhere off-site immediately, so that you don't suffer the headaches of data loss. Afterwards, I'd advise you to check its warranty status. If it's relatively new and still covered by the limited warranty, you should be able to RMA it and get a replacement from the reseller's or the manufacturer's customer support.
This is the only way to solve the issue as tampering with it physically will void the warranty and you won't be able to fix it.
