HDMI input not found while playing games and error sounds

So the history of this issue. All started when I downloaded the new expansion pack for Witcher 3, hearts of stone. But nomatter what game is played at one point during the gameplay these things happen in this order: (Notice: This all happens within 3-4 seconds)

1. Monitor shows that input HDMI 1 is not found (this is what my graphics card is connected too on my monitors input.)
2. Sound of game stops
3. USB hardware insert sound (https://youtu.be/ufKjjgvQZho?t=969)
4. Windows volume/background sound (https://youtu.be/ufKjjgvQZho?t=953)


Things I have tried:
1. Reinstalling the OS
2. Formatting all hard drives/SSD
3. Reinstalling the graphics card driver
4. Virus removal (found 15 malware but problem still exists)

All temperatures are normal no overheating
Computer runs fine other wise. `ONLY does this when playing any game, but this pc is built for gaming so other uses for it are limited.

Also on a side note this could be the problem but not sure. 24 pin power connector from the power supply to the motherboard cant be plugged all the way in. Yes this is a problem but would this cause the graphics card to stop output. I think this is just a coincidence.

Ok so I checked the power cables to the GPU and they seem to be fully intact. Buying new HDMI cable so I will get back to you on that. Also I found that when the PC is on its side, (Processor facing up wards) the pc runs fine with no errors.
 
Remove all the cards and re-seat them. If you can get to the CPU easy, do that too and all the RAM. Remove all cables pluggged into the Mobo and re-seat them also, Sounds like you have a marginal connection somewhere.
 
Ok so I followed what @Wiz33 told me to do. I unplugged everything (except CPU and CPU COOLER) and I re plugged everything back in. 24 pin is still not all the way in but it doesn't matter cause now my pc wont crash any more. So to conclude there was some type of loose connection somewhere that was causing this mess. THANK YOU!
 
Ok guys guess what that did not solve the issue. I did some research and found out about windows logs. I tracked down the log that makes my pc crash and I get this error. If someone can guide me into the right direction of how to fix this, it would be great. Thanks!
