Please, Please, help!! I've been trying my best to understand what is wrong but I can't figure it out :/

Is this overclocked? Have you run MEMTEST? What exact motherboard and power supply ?

Not overclocked, I haven't run MEMTest because I don't have an external hard drive or any cd laying around.

Motherboard: MSI 760 GMA - p34 (FX)

Power Supply: EVG 500 B

Never! I built this PC in 2016 but have never done a clean install. :/

MEMTEST can be put onto a USB drive and booted that way.
