Question (Help) Computer loses connection to monitors then stalls/turns off ?

A lot of the times when I boot up my pc and when im using it the pc freezes,monitors go dark saying no HDMI signal then the pc wont do anything until I restart it.
Once I do restart alot of the times I have to "fix" my hard drives.

I've had this problem for months as well as the occasional video_scheduler_error.
I've tried taking out and putting back in my ram. Swapped out my entire power supply and wires with new ones.
Only other issues I have are probably outdated drivers/bios.
Slightly broken cpu fan.
and my hardrive is at 100% a lot of the time.

Any help would be appreciated if it could get rid of this, its been plaguing me for months.

Update your post to include full system hardware specs and OS information.

Hard drive: make, model, capacity, how full?

What do you mean by "slightly broken cpu fan"?

Open Reliability History and look for any error codes and warnings that you can associate with the PC freezes, etc..

Look in Resource Monitor to determine what is using the hard drive and to what extent (%).
