Help me, HDD possibly failing?

I've been having trouble with my PC for some time now. I think something is wrong with the HDD. It BSODs out of nowhere and if it doesn't, then explorer.exe crashes, and won't restart, even if I try to run it myself, it won't open, then anything left will close down after a few minutes, and then the PC will just reset. I've ran CrystalDiskInfo to check the status of my HDD and it says it's at risk, Current Pending Sector Count is on yellow and so is Uncorrectable Sector Count. Anyone have any idea how to fix this? I've been considering changing the HDD but there's probably a way to fix my problems.
 
Solution
It seems quite clear, the drive is going to die. Change it.
If you want to try to give it another life, you may try a low level format erase with HDD LLF Low Level Format Tool from HDDguru...it is free...then run another diagnostic to check...
