HELP! Random Grey Screen of Death ?

Please let me know if i'm in the wrong form and I'll be happy to delete this post and re-post it into the proper sub-form. I've been experiencing a sudden occurrence of crashes while in game. At first I thought it was just ARMA 3 being buggy with the new 64-bit client but then I was able to reproduce it while playing Rust and BF1 as well. However, I've never experienced the Grey Screen crash while playing League of Legends(weird? doubly so considering I play this more than any other game).

When ARMA 3 grey screens I get the message :
"Application has been blocked from accessing Graphics hardware" but on occasion I've also been given the error messages:
“Unusual process exit” +
"0xC0000005 - ACCESS_VIOLATION" or "0x00000001 - UNABLE_TO_INIT_DXGI"
if I am able to alt tab/ctrl+alt+delete fast enough to avoid getting stuck in grey screen and having to shut down

I've followed the recommended solutions for these problems recommended on "https://community.bistudio.com/wiki/Arma_3_Unusual_process_exit" and while I'm not getting those specific error messages anymore the error message "Application has been blocked from accessing Graphics hardware" persists.

solutions I've tried:
Rollback latest windows 10 update
Rollback latest Nvidia Update
reinstall Visual C++ 2013 Redistributable
Verify the integrity of the game cache using Steam
Run a Windows System File Check tool to repair corrupted system files.
Disable the Steam Overlay.

I do not get the same error of "Application has been blocked from accessing Graphics hardware" in windows notifications when Rust or BF1 Grey-Screens. My computer doesn't seem to hang and while most of the time I can't ctrl alt delete to open the task manager and avoid having to shut down if I then press the power button my grey screen will switch to a "shutting down" screen giving me the impression this is some kind of problem with a driver/ direct x/ GPU. Also, I never grey-screened on startup which seems to be most peoples issue with grey screens. I've also never grey screened while in a games server browser/UI(I let the main menu of BF1 run overnight) only while in-game and at seemingly random times(sometimes I crash after 30 seconds of being in-game sometimes I can play for an hour before a crash occurs)

My CPU and GPU are overclocked however up until 48 hours ago they appeared to be very stable. What can I do to monitor my computer to narrow down the cause of these Grey Screens. I've heard these grey screens can be caused by faulty V ram or a failed overclock but I've had the card for years and my overclock was stable for over a month now until this problem happened so I'm not sure if these things are the culprits. Any advice on how I can monitor my computer to narrow down the problem would be greatly appreciated !

Here are some things I suppose might help(let me know if I should include anything else):

My settings in EVGA Precision XOC:
Power Target : 112%
Temp Target : 80C
GPU Clock Offset : +150MHz
Mem Clock Offset : +500MHz

I have GPUZ CPUZ HWMonitor RealTemp installed for monitoring and I just enabled the logs in HWMonitor and GPUZ.

F.LUX(which I disabled and reproduced the grey-screen)

i5 3570k @ 3800MHz(yes I know in theory I should be able to get this much higher but that's a whole other issue)
