Help Upgrading My Existing PC

Hey people!

So my PC was built for me about 5 years ago, and it's served me really well but it's about time for an upgrade. I'd ideally like to be able to play GTA V and other new games on high settings, I also use my PC for video production and photoshop, etc. I was wondering if I could get some advice if it's smart (or even possible) for me to try and upgrade only a few parts of my PC, or if it's not worth it and I should try and build one from the bottom up.

Here's what I know about my PC, as of now:

CPU: Intel Core Duo E8400 @ 3.00GHz
4GB RAM
ATI Radeon HD 5570 2.8GB

My motherboard is a MSI 7529

Power supply: Power man 300W (http://goo.gl/TJ3oca)

So basically, my questions are: With this motherboard and power supply, would I be able to upgrade my cpu/ram/graphics card? Or are the mb and power too outdated to make such upgrades.

I really only have about $400-$500 to spend and am not sure what my options are.
