Help with NVIDIA GPU error 43

So currently I am using the integrated graphics of my computer, and whenever I connect the monitor and I set the GPU to the GTX 760, I get an error that says "Recovery, Windows didn't load correctly" and I can't start my computer. However, with the integrated graphics everything is fine, and when I find the GTX 760 in device manager I see this:

"Windows has stopped this device because it has reported problems. (Code 43)"

I tried uninstalling and reinstalling the drivers, which worked and said that the device is working, but after restarting the computer and using the dedicated GPU the same problem returned.Also there are some small blue blocks covering the screen whenever I use the it.
Can you recommend me some ways I could diagnose the problem? and is it possible the GPU is dead for good?

Code 43 is either device driver problem or hardware problem. Was that GPU working before on your machine?

yes it was working fine for a few years but suddenly when I was playing a game the screen froze and I had to shut it down

In that case, if reinstalling drivers didn't help, most likely it is dying GPU.
