How bad is the C5 and C6 value right now? Can i fix this in any way?

Seems like you are having some bad sectors
As of now the whole drive is suspect, it won't improve from here on out

My advice would be to backup and replace that drive, especially since it's a hdd and those come cheap

As others have said, that is bad news, especially if those numbers continue to rise. I would move the data off that drive to a different one (that is in good shape) ASAP, and replace the drive.
