When it was working:
GeForce 960 4GB SSC

DisplayPort to HDMI cord (no adapters just the cord)
LG 25UM56-P Ultrawide Monitor with cord above

HDMI to HDMI cord
VIZIO 1920x1080 used with cord above

After the update:
2560x1080 is no longer an option
creating custom resolution 2560x1080 creates black bars on top and bottom of screen and makes everything blurry.

spent last 3 hours trying to get it to fix and I have to go to work.
Thanks to anyone who can help.

Solution
Remove all of the Nvidia related items by way of either DDU or through the Windows Control Panel. Reboot and see if anything changes. I know that My Geforce Control Panel option disappeared from my right click options and I figured it had something to do with the display driver update I just downloaded. I did the control panel route and uninstalled everything it would let me, restarted, and then it was there to use again.

My Geforce Experience is still broken, however.
After a recent update, my Nvidia Geforce Experience stopped working and cannot open. It throws an error. I suspect your issue is somewhat related. As Andro suggested, you can go the DDU route but be careful and read ALL of the instructions so as to not nuke something along the way and make matters worse.

You can't see the resolution at all in the Geforce Control Panel?

right. it's not an option in anything
