High fps in Minecraft, but it still looks very choppy (MicroStutter?)

I get perfect fps in minecraft, but it looks comparable to playing with 15-20. Without v sync on i run at 400 fps, which is way above my monitors refresh rate. Usually this would be an issue of un-synchronization so i would turn on my v sync. After v sync is on it gets even worse, and by bad i mean "I cant move my character" bad. Even though it says im getting 60 fps the whole time. Ive tried every trick in the book, (v sync, lwjgl update, java update, more ram, less ram, overclock, underclock, forced adaptive v sync, triple buffering, and optifine). None of them improved my issues. At this point this game is completely unplayable. Id prefer to be able to play with v sync on to save power but it just makes it worse. Doesn't happen in any other games, i can run Portal 2, WoW, Diablo 3 , Starcraft all on max settings with no problem.

My computer:

I5 3570k at Stock
8 Gigs of ram at 1600 mhz
Evga Gtx 460 superclocked 1g
Minecraft installed on 128g Ocz Vector ssd

To add to the question, I think that my gpu is possibly dying. Would that have any impact at all? Somtimes i boot up and i get an 800x600 screen with about 4 colors and tons of artifacts. Also had some bsods earlier on, but that may not be related. Its a refurb that they sent we when the first one bit the dust.
 
do you play other games okay? if not its your gpu. otherwise its just micro stutter. i used to run minecraft vanilla (no mods) with twitchy performance for some reason. now i run ethers unbelievable shaders mod, 256x256 texture pack, and optifine and get 50 fps stable. if your on a multiplayer server with tons of people then its almost certainly that, i only run servers with a few friends so i dont run into that issue. if this is happening in single player go to optifine and there should be a setting called smooth fps in one of the menus. try that it helps my buddy with an older computer play at 24 fps but it feels like 40. in optifine there are a lot of cool settings to improve performance when running a lot of mods

delete minecraft and reinstall. also try technic launcher, if that runs smoothly then perhaps use it? it has vanilla minecraft along with a lot of mod packs
