Hoping for some help upgrading my HP desktop..

Greetings all. New member here, but I've been reading the site for years. Despite that, I'm still pretty clueless in regards to PC building, so I definitely appreciate any guidance on the topic.

Specs:
Intel Core i5 3470 (3.20 GHz)
12 GB DDR3 2 TB HDD
Windows 8 64-Bit
NVIDIA GeForce GT 620
120 GB Samsung Evo ssd as main drive
2tb hdd as back up drive

Thus far, the pc has been great, and met all of my needs and then some. I use it for internet browsing, some light photoshop, and occasional video editing, while using my PS4 and 1080p plasma for gaming. Life is swell. I've always been a satisfied console gamer, but this Acer IPS, 21:9, 3440x1440p, freesync, and 75+ hz, is giving me the tingles and has me ready to make the leap. Ascend, if you will. Ideally, I'd like to be able to get 50-75 fps at max/ultra settings on most new games.. BF4, Witcher 3, Arkham Knight, etc.

Obviously, my current set up won't get me there, and my initial reaction was to build a completely new system. After a bit of research though, it seems my current PC has some decent hardware well worth reusing - Although I could easily be convinced otherwise. With that in mind, my current plan is to upgrade to a new case, a new psu, and gpu. Also, I'm guessing I need a better cooling solution.

Undecided on the case, psu, and cooling solution so I'm all ears if anyone has any suggestions. Also, I have a few questions I'd appreciate some help with:

a) Overall, does that sound like a solid plan, considering my goals?

b) Should I be concerned about any potential compatibility issues with a HP (I assume) proprietary motherboard and the r9 fury?

c) Will my i5 3470 bottleneck the r9 fury?

d) 12gb of ddr3 is enough, right?

Basically, does anything stand out in my plan that isn't going to work? I'll be spending well over 1k and really want to make sure I get the most out of it!

Specifically this part:

one thing I seen and learned with prebuilts is the boards don't have to meet atx standard and like the pci-e slot may not be set to the full 75w a higher end card requires to work ... as most find out so it wount matter if you go with a 5000w psu the boards fixed slot power will stop it from working [ most put the pci-e slot power at 45w ]

Ok, so I'm not gonna act like I know everything because I don't, but I would say that if there were to be a bottleneck between that i5 and the fury it would be minimal and 12gb if RAM is more than enough. I would also like to let you know that Arkham Knight is HORRIBLY optimised on PC, you would be better off just getting it on PS4.

And about your plan sounding good, sounds great, I have a similar System, same processor, and I would love to do something like that. Compatibility wise I'm not sure though - your PSU may not have the required connectors and your case may not have enough space, a good place to check would be the HP forums to see if anyone else has had the same dilemma.
