Horrible benchmarks & glitching with gtx 960 PLZ HELP!

so i just finished a build here is what is in the build:
1) nvidia gtx 960 2gb
2) 1tb 7200rpm sata lll 6gb hdd
3) i5 2400
4) 8gb corsair vengeance 1600Mhz ram
5) Asus P8H61-M LE/CSM mobo
6) zalman cpu air cooler
This is what im experiencing, after every build i do i run a benchmark test with heavens, my test on this build is glitching from the start and i get a 585 score with a high of 35 fps, this is a problem, this tells me something is wrong, i did a fresh install of windows, everything is updated, windows tells me the gpu and cpu is working fine, i swapped the cpu out to another and the same thing, the score should be around 1500 on heavens for this system. Im completely stumped, ive read as many forums as i can find and nothing to explain my issue. is this particular card not compatible with win 10 maybe? any suggestions would be great! thanks in advance
 
Assuming you've installed all the latest drivers for the mobo as well as for the GPU, I have the following questions:

- Which PSU do you have?

- Have you made sure the extra PCI-E power connector is properly plugged into the GPU?

- During the benchmark tests, what was the CPU and GPU usage %, their temps and their frequencies?
