how do cards occupy two slots? I though you plug them into one slot?

GPUs only use one slot electrically-speaking but their heatsink, fan and rear IO/exhaust usually takes two slots. Some cards have HSF so thick they use three slots.

Yes, lots of cards are wide enough to block a second slot. The heatsinks and fans on high-end cards are large so they can dissipate a lot of heat..

ok im not worried cuz ill use a riser anyway. also it reduces temp and some risers are powered by molex too.
