How do I find RAM serial number and model number for RMA?

Hey all,

My PC was constantly crashing so I ran memtestx86 on each of my RAM sticks and found that one was faulty. I am now trying to fill out G.SKILL's RMA Request Form, but I do not know what to put in the value for the first two questions (here is the page: http://www.gskill.com/en/rma). How do I locate the serial number and model number for my faulty RAM stick?

There should be a label affixed to the memory sticks that contains the info.
