How do the CCG's/TCG's define "Broken"?

hey,

I'm wondering how the various CCG's/TCG's define "Broken". This
question includes "Broken Cards", "Broken Combos", and/or "Broken
Rules". Possibly even certain games are "Broken".

1. What is the game?
2. Does the game have a specific definition of "Broken"? What is it?
3. How does the game determine if something is "Broken" (playtesters,
judges, etc ...)?

In YuGiOh (I am guessing in Magic also) Broken means the card is very
powerful and can be used in almost any situation that the card effect can be
used and doesn't have many if any drawbacks.
I don't think the term makes much sense but it's the term that is part of
CCG vocabulary.
