How to connect power to graphics cards

My graphics card came with a 4-to-6 pin adapter. Should i connect both 4 pins or just one to the PSU.

Also need to know the brand and model of the power supply. In general, if a power supply does not have a PCIe power cord, it's not a good idea to use it with a video card that needs one.
