When you install the game all you should have to do is change it form the C drive to the D drive when it ask where to install it. Sometimes you can move a game after it has been installed and point the shortcut to the D Drive.
if you have like Steam or Origin you can move the folders to the other drive and then reinstall Steam and Origin on the D Drive installing over what you copied. it should pick up the games that are already there and not have to redownload them again.
For Origin make sure you copy the Origin folder and the Origin Games folder.
You don't have to put them in line D:\Program Files\Game Folder ect. They can be installed right on the D Drive or what I like to do because i don't like cluttered root drives i make it...
