HP Slimline Upgrade...

First of all, good day to anyone who reads this. I got a HP Pavilion Slimline s5310f has a NVIDIA GeForce 6150 SE graphics card (also supports PCI Express x16 graphics cards) and either integrated graphics or the PCI Express x16 slot are usable at one time; they are not usable concurrently. Also it has a power supply of 220W. Here's also the list of the specs of my computer (is that the right term to use?).

From all this info I wanna know what's probably the best graphics card I can fit in it since the tower is pretty small and for a good price. I plan on running Skyrim and Borderlands 2 on my PC on max settings and I know it can't even do that right now. Let's also say hypothetically I want to run a game like Battlefield 3 on it, what would I need to change? I know for a fact the graphics card and PSU are gonna have changed but anything else to make my life easier with my gaming time (meaning even if a new game comes out and its pretty heavy on graphics).
