Unfortunately, it's not as easy as adding a new graphics card because the CPU is very old meaning you need a graphics card, a new CPU, a new motherboard for the newer CPU, and ddr4 for the newer mobo and CPU. This is more expensive than just buying a graphics card but possibly worth it. I put together a couple of different list of parts I would recommend but be sure they with all fit in your pc. You may also consider a better power supply;https://www.newegg.com/Product/Product.aspx?Item=N82E16817438145&Description=evga 500w&cm_re=evga_500w-_-17-438-145-_-Product

Cheaper but using integrated Graphics so it doesn't perform as well: https://pcpartpicker.com/list/b2cLfH (Look up 2400g benchmarks on youtube)
More expensive but much faster: https://pcpartpicker.com/list/Q2LGZR (Look up Rx 570 4gb benchmarks on youtube)

This is the issue Ive been having, I cannot get anything to run right. No game will run well and Ive upgrade the ram to the highest amount. I can defiantly add a better power supply and Processor.
Radeon RX570 4g Graphics Card I just installed.

It's your CPU causing poor performance, if you can reuse the Case, PSU (if good quality 550W unit), Storage & GPU then I would get this as it would be a massive improvement over your current system. You'll want to do a clean install of Windows then download the newest drivers for the motherboard & GPU.
