I Need Advice On Upgrading my PC

Ok. So, i have been running on a pretty decent system since '11, and it has kept me afloat so far.
Specs:
Intel i3-2120 CPU @ 3.30GHz
4 GB of Installed RAM
ATI Radeon 4850 512MB Dedicated Memory (2.2GB total memory) Graphics Card
2 1280x1024 75Hz monitors
Motherboard is a preinstalled Dell Baseboard model number 0M5DCD
Tower is a Dell Optiplex 380 Mini Tower
500GB of disk space, 1TB external drive storage
(I tried to make that as extensive as possible, sorry for the length.)

I will have approximately 300~ USD around new years time, depending on circumstances, so any current sales/mail in rebates wont be of much use I dont think.
I found this guide to build a (what looks like) sweet PC for the latest games.
I, unfortunately, don't have the kind of money right now.
My question is, can i pick and choose from the aforementioned guide, and will any of those parts work with my current setup (as in Add on/Replace).
Failing that, what would you recommend as good upgrade parts capable of running current Triple-A games (I.e. Fallout 4, Gta V, and possibly skyrim with some graphics mods)?

You should definitely upgrade your GPU. Your CPU is starting to bet a bit dated but good enough for most games. I suggest getting an R9 280X or an R9 380. You may need a new PSU. And also get some more RAM. Does your motherboard support DDR3 or DDR2 RAM?
 


My motherboard can support DDR3 Ram. It doesnt support Multiple graphics cards support so i cant keep my old one for my second monitor which sucks. I dont think that i quite want to spend ~200$+ on a R9 280X or R9 380, especially if i need a new PSU and RAM. I found an R7 370 for 160$, do you think that will do for mid range graphics? Or is the extra 40$ really worth it? Also i dont know what PSU I have but if i need one what would you recommend? Im sure i can find some 2X4GB ram for cheap but do you have a recommendations? Thanks a mil, and im sorry im asking so many questions, im new to pc building...
