I need to buy some black shelves that are 14 inches wide, 11-12 inches long and around 5/8-3/4 of an inch thick. Can anyone tell me where I can buy a shelf like that?

Solution
I take it that you have something that you need to install a black shelf 3/4" x 12" x 14" into and you just need the shelf itself? If that is the case, I'd take the retired airman's advice and use a piece of plywood. You can get melamine-coated plywood shelving at most home stores and black is often a color they carry. It's generally coated on all four edges, so you can cut it to length (one side) and rip to width (one edge) and still have the front edge coated with the melamine. All you would need to cut it is a circular saw with an edge guide or a cheap benchtop table saw. If you have ever used a powered saw before, you could perform those two cuts and be done as cutting a small piece of sheet good to size is simple.

I would not go...
Plywood and a can of black paint?

Recommending 'where' would require someone knowing where on the planet we're talking about.

Maybe this, and take it apart

I would not go the "get a piece of regular plywood and paint it black" route as you will either have to edge band it or stare at an ugly exposed plywood edge. Edge banding plywood is not difficult but you will at the very least need a router with a flush trim bit (top bearing) to trim the edging to the correct width after you glue it to the plywood.

If you are talking about needing a set of shelves about that size, then you can either try to look for one online such as on Amazon. I am guessing that
