I suspect my graphics card to be dying

Problem: Glitchy screen graphics and occasional crashes. I have included pictures since I find it hard to describe what is actually happening. Occasionally the video card will just crash, forcing a hard reset.

When playing video games, sometimes the screen will glitch. Usually happens after an hour of gameplay. The problem is not solved by exiting out of the video game. In addition, I have experienced this problem through several different video games, but never when the computer is idle.

If anyone can diagnose what is happening to my computer, that will be very much appreciated.

Specs:
Motherboard: TZ68K+
OS: Windows 7 64 bit(6.1, build 7601)
Processor: Intel Core i5-2400
Graphics card: AMD Radeon HD 6870
Hard drive: ST1500DL003-9VT16L ATA Device
Power supply: SS-620GM

Attempted troubleshoot actions:
- Rolled back drivers, also reinstalled drivers
- Removing the HD 6870 for dust cleaning, and re-inserting it
- Google
- Restarting the computer, works until I decide to have fun again
 

The CPU is very likely to be the issue, is the heatsink seated properly? Does the fan work?
Like Mrcreballing suggested, you should try replacing your thermal paste.
 

Make sure to keep an eye on the temperatures just to keep your parts safe :)
