i5 4430 processor underclocking itself

For a few days now, my processor is underclocking itself causing horrible lag during games, youtube, skype, and just internet browsing. Using CPU-Z, I can literally watch my CPU Core speed change from 3ghz, down to 700mhz and every number inbetween. Again, it does this during idling, gaming, and just browsing, and the drops are noticable, as in games such as Arma III on High, I go from a steady 80 down to 20 with terrible stuttering and the sound gets crackly. I also notice my Core voltage varies from 1.055 down to below 1.0v. Any help is appreciated.
My rig is:
Processor: i5 4430 3ghz quad-core
GPU: Asus gtx650 2gb
Ram: 8 gig G.skill Sniper
Motherboard: ASrock B85m Pro4
Power supply:EVGA 500w
 
Thank you to everyone who tried to help. I figured it out. Followed everyones help here, and nothing worked, until I checked my cpu cooler, somehow one of the pegs holding it in came loose causing it to no longer cool my processor properly. After checking HWmonitor I was running 94C. I shut down my PC and re-attached the cooler and now everything's good as gold.
