Image tearing problem while playing games

I built a PC recently specs are:
Ryzen 1700 (OC @ 3.875, cooled with NH-D15)
GTX 1080
Gigabyte AB-350 Gaming 3
16 GB RAM (ddr4, 2666 Mhz)
500 GB SSD / 2 TB HDD
AOC 28 u2879vf (4k, 1 ms, 60 Hz)
Antec 900w 80+ Bronze

I just finished playing Wolfenstein The new order and installed GTA 5, I was facing few tearing in Wolfenstein, but it got worse in GTA. I play in 4k with ultra-high setting (as high as possible).
Now, can anyone please help me to remove this image tearing. It is spoiling my gaming experience.

Already tried, not solving the problem, plus decrease in FPS.

Adjust your settings so that you're getting consistent 60+ fps, and then adaptive sync or quick sync in nvidia settings.

Edit: Adaptive sync will result in some screen tearing if you drop below 60 fps.

Already tried, not solving the problem, plus decrease in FPS.

If vsync doesn't work then you could have some serious issues. You should first remove the driver with DDU then install it again with the least amount of stuff. Only driver and physX. If you use sound though the monitor then the nvidia audio driver as well.

Should I reformat my windows? (also want to let you know i changed my Monitor and upgraded from rx 480 to gtx 1080 and didnt uninstall any drivers)

nah just use DDU to remove all the drivers. Make it remove amd and nvidia ones then install the latest nvidia driver with minimal options (just driver and physX).
