Recently, I've been trying out a 4.0 GHz clock on my 4690k. Under full load in prime95 (default blend test) there is an enormous temperature spread between the cores: 91-80-60-50. Link to hwmonitor readings: https://image.ibb.co/caDAKp/test.png. I have never heard of that kind of temperature spread before. Could this be attributed to an uneven thermal paste application? If you have any ideas as to the cause of this, I'd be very happy to know.

That's a pretty savage spread, no doubt.
As artk2219 suggests, pop off the cooler and check the TIM spread, you may even spot a loose screw or screws during the removal, if any are loose, tighten them first and recheck the temperatures.

Uneven spread, bad paste under the heat spreader, or you just have a hot core, those happen from time to time, either way it looks like you have a prime delidding candidate to investigate that with. But I would start with making sure you have a good thermal paste spread on top of the heat spreader, and good cooler contact all around.
