Internal USB 2.0 and 3.0 on single PCI card

I am looking for a pci or pci-x or pci-x16 card with internal usb 2.0 header and internal usb 3.0 connector for front case devices.

Does you motherboard have NO front panel adapters, even for 2.0?

If you NEED to add 2.0 headers, you need two separate items, one that adds an internal USB port, and one that converts that port into a header:
