Is it time to replace my Hard Drive?

So i bought a computer around 3-4 years ago and ive been upgrading it alot recently with things such as a gtx 960, i5 4460 etc although in that time ive used the same hard drive although ive noticed especially in the last few weeks that when i play games for awhile my game starts to get extremely "choppy" for example in gta v when i drive to a new area the fps drops to like 5 for a few seconds, in minecraft when i try to move (in my own server) the fps drops to around 5 as well and if all that wast a big enough hint when i look at my task manager in these situations my disk usage is at 100% and say when i just start my computer or am not using it very often it rests at around 70-80%

If your boot disk is a traditional HDD, yes, upgrade
