Is my fan hub splitter PCB shorting out the fans?

I'm building an ITX build for a media centre/mums little PC and I needed some extra pins for the rear case fan. I purchased a molex fan hub as the PSU I'm using is from an old build. I plugged the splitter it into the molex and tested the first fan and it didn't spin. So I grabbed another one I had spare in the box, it spun up but as I was managing the cables I heard a spark and the fan stopped spinning. System works fine, posts and works. What did I do wrong? Is there a part of the splitter that cant touch the case? Did I manage to short it out? I used Corsair SP 120 quiet editions (my favorite fan) and Phobya 4 pin to 6x 3pin fan splitter PCB.
Help would be much apreciated, this is only my 3rd PC build and first ITX build so I'm still very green.

On each of the six 3-pin fan connectors of that hub, the central one is the +12 VDC pin. NONE of them should be allowed to touch a Ground.

Oh thanks for the info. So I must have touched the pins to the case which would have grounded them?

Yes, that would do that. Now, I'm not clear whether your fans and hub are working again, or not, If there's still a problem, post details. Thanks for Best Solution.
