Is my GPU dying?

Hey guys, i'm having some issues with my PC currently and it is driving me absolutely insane. I'll get right down to it.

First of all, some issues i've been having are my monitor disconnecting from my PC randomly. So basically, play for about 30 mins to an hour. It varies, and all of a sudden my screen will disconnect and say DVI not connected. It's clearly connected and it just won't let me do anything on my computer. Computer is still on, makes the Win 10 login/update sound and my audio is still working perfectly fine. Hard reboot PC and screen is back on working fine. As of right now this problem isn't currently happening, dusted out my system and such, but that doesn't mean it won't happen again. This was also occurring only when I attempted to get into my PC.

Another thing it's doing is completely freezing my PC when i'm gaming. It only happens whenever I am gaming. It is driving me nuts.

Now sometimes i'll be able to go maybe 3-4 hours before it happens but a lot of the times i'll barely be able to play for an hour. It's very frustrating as it only happens whenever I play games. Besides that, i'm able to watch videos and do my regular things on my PC just fine. It just happens whenever I am gaming. I plan on replacing the GPU as that's the only thing I can suspect but I really would like people who actually know more about PC's to help me out.

Solution
Do a clean install of the drivers (use DDU to uninstall). If that doesn't help, plug the monitor into the back of the motherboard and take out your graphics card. If the problem goes away then yeah it's a problematic GPU.
