Is my graphics card dead?

I believe my graphics card is broken. Every time I start my computer, I get horizontal white lines across my screen. To fix it, I had to reseat the video card up to 10 times in order for it to work. Now, not even that works any more. It is done.

I have tried everything to fix it. I think it is time to RMA it. It is still under warranty. Here are the thing I tried.

>I cleaned out the fans
>Properly cleaned out drivers with driver sweeper programs
>Switched pci-e slots

Any help would be greatly appreciated. Thanks!

It is an Asus GTX580 directCUII

Yeah send it back, but try another PSU first, it may be causing issues.
