Is my graphics card dying?

I was playing a game and my display drivers crashed, un recoverable, had to hard reboot.

Kept happening! only now, the game would fail to start!

I can boot into safe mode. I Can boot into regular mode with base video, but now when doing that I've even started to get horizontal black lines.

Currently have the display drivers disabled, windows will boot.

How can I diagnose the source of the problem, Will a need a replacement video card?

Mb: gigabyte x58a-ud3r
CPU: i7-930 (no o/c)
GPU: powercolor 6870
PSU: 700w stealthxstream (3years old)
SSD: ADATA s510
HDD: 1tb hitachi 7200spm, 2tb hitachi 5400rpm, 3tb seagate


 
My PSU was on the top of the "To be Upgraded list" I suspect that it is past it's prime. My system is on nearly 24/7.
