Is my graphics card dying?

Hey guys, so for about the last month my PC has been booting up with my Z77 extreme on-board intel graphics, instead of my HD 7970. In the device manager I see the card grey'd out with a code 12. I've tried setting it to boot from PCI-E in the bios, uninstalling the on-board graphics, and completely clean installing the correct graphics drivers. It's gotten worse lately, the last couple of days I've turned on my PC to black screens, and straight to the bios. The card is about 3 years old, I need a way to be sure it is or isn't the card. I'd like to sell it to put in a little bit towards a 1070, but I can't without knowing. Thanks for any help in advance.

Can't right click it in device manger and uninstall and delete drivers? I'd try it on anther system as suggested above to rule out the mother board being an issue. Also try another PSU, maybe its not providing enough power to the card.
Those Radeon cards end up dying.
Your lucky to have it that long.
But, i may be wrong. Brng it to a pc store if you can, or a friend's house.
Check the bios for voltage fsctors. Or perhaps there maybe a utility to monitor it possibly
