Is My Graphics Card Failing?

I built a new computer and its been working great for me. However, I noticed during more intensive games it will suddenly crash (goes straight to black screen). I usually reset it, and its back to normal.

I realize this can be due to a variety of problems, but I believe it is due to the graphics card (the graphics card is the 1 part that wasn't brand new when I built my computer.).

I ran a FurMark Stress Test and recorded the findings using HWMonitor. The test ran for 7:50 minutes before crashing. The temperature of the graphics card shot up from 35C to 85+ within the first 2 minutes. Based on these results, am I correct to assume my graphics card is the problem? I also believe it could be a power supply issue. *I have marked noticeable differences with a asterisk*

MSI Motherboard
- Voltages: consistent
- *Temperature: Consistent, except SYSTIN increased from 33C to 43C. TMPIN3 increased from 34C to 38C
- Fans: Consistent

Intel Core i5 4670K
- Voltages: Consistent
- *Temperatures: Core 1-4 and "Package" increased by 4-5C
- *Powers: Example: (Min, Max before) and (Min, Max After)
-Package: (0.72, 9.78W) Before and (0.78, 5.45W) After
-IA Cores: (0, 6.64W) and (0, 3.47W)
-Uncore: (0.42, 4.14W) and (1.78, 2.98W)

WDC (Harddrive)
- Temperature: Consistent

ATI Radeon HD 5750 (old graphics card)
- *Voltages: Increased from 0.950 V to 1.1 V
- *Temperature: increased from 37C to 92C. Crashed at 92C.

Based on this test, what seems to be my problem? Is it indeed the graphics card? It would appear that way from the test, but I know very little about computer hardware
 
Solution
92C is extremely hot and the GPU is probably shutting down at those temps. One remedy would be taking the heat sink off, cleaning it completely of dust, than reseating it with new thermal paste. Good luck
