Is my Hard Drive failing?

Lately the performance of my computer has been really slow and sometimes not booting into Windows until I fix it. I can normally play gta 5 at very high settings at 60+ fps but sometimes I have played it for 30 seconds then it freezes and then doesn't respond, even a game like tf2 is normally at 200+ fps but dropped to 60-70. I have also been starting to hear a clicking noise and a grinding/buzzing noise which I assume both to be the hard drive so is it starting to fail on me?
It's a Toshiba drive if that helps somehow.

New noises are never a good sign. However, we can't automagically assume it is the HDD.
Probably is, but not 100%.

A rolled up piece of paper held to your ear, and pointed around in the case might help localize the sound.

Get a backup now! Even if it's not the HDD - it can't hurt

Clicking noises are definitely coming from around the HDD but the noises are so inconsistent, one time it could be clicking a fair bit then the next almost not at all.

That drive appears to be dying.
Backup whatever is on it, and look towards a replacement.
