[SOLVED] Is my HDD dead?

Hi recently my HDD is freezes at destkop and programs. Sometimes it causing reboot system. I've checked bad sectors with HD Tune and it found 3 block bad sectors. I've tried chkdsk C: /f /r command but it doesn't solved problem. At file transferring i've seen sometimes transferring speed stucks at 0 byte/s and increases at 20 mb/s avarage. My HDD 12 years old. I think it might completed it's life.

CPU:Intel Xeon X5450 OC 3.44GHz
Mobo:Asus ROG Commando P965 Chipset
RAM:Kingston 2GB DDR2 800MHz CL6
HDD:Samsung HD250HJ 250GB
GPU: AMD Firepro V5800 1GB GDDR5
PSU:Hytech HPS-1560A 280W

I think you are correct.
I think it is definitely showing signs of failing.
Those are all signs I see with bad drives.
Plus...it's 12 years old.
I think it's time for a new one.
